Full plot (contains spoilers)
Years after the Rapture, humanity is hunted by the Burned Ones, demonic creatures drawn to human flesh. Azrael and her lover, Kenan, are ousted from a silent cult that surgically removes vocal cords. After being captured by the cult leader Josephine, Azrael escapes a sacrifice attempt and flees into the woods. She encounters a man in a truck who is killed by a cultist, and later finds Kenan nailed to a tree; both are killed by the Burned Ones. Azrael returns to the cult's encampment, confronting the spiritual interpreter Miriam. After a struggle where Azrael injures Miriam's pregnant stomach, the cultists bury Azrael in a coffin. A Burned One descends but retreats upon sensing Miriam's blood on Azrael's hand. Enraged, Azrael escapes the casket, sets the encampment on fire, and kills the members one by one. She finally confronts Miriam in a church, biting her neck. As Miriam goes into labor, Azrael kills Josephine. Miriam slits her own throat before giving birth to a goat-like creature resembling the Antichrist. As the Burned Ones enter the church wailing, Azrael stands holding the newborn baby.
