The story
Nick Cavanaugh, an Atlanta surgeon, moves into his late mother's lavish home with his girlfriend, Anne. However, Nick remains fixated on his neighbor, Helena, a woman with whom he shared a single intimate encounter. When a housewarming party leads to a falling out between Nick and Anne, and a subsequent accident leaves Helena injured, Nick takes her into his care. The narrative follows the escalating tension and physical confinement of Helena as Nick's obsession manifests in extreme actions to keep her under his control, while she remains defiant in her circumstances.

Nick Cavanaugh, an Atlanta surgeon, moves into his late mother's home with his girlfriend, Anne. He is obsessed with his neighbor, Helena, a woman who treats him with disdain. After a party where Anne leaves him due to his fixation, Helena is injured in a hit-and-run. Nick kidnaps her and provides clandestine medical care, eventually amputating her limbs to ensure she remains dependent on him. Despite her physical state, Helena remains verbally defiant. The situation escalates when Helena's boyfriend, Ray, finds her and confronts Nick in a violent altercation. It is ultimately revealed that the entire ordeal—the kidnapping, the mutilations, and the confrontation—was a vivid nightmare Nick experienced while waiting at a hospital for Helena to recover from surgery. He wakes up in a hospital bed, still haunted by his obsession.
