Full plot (contains spoilers)
Chev Chelios survives a fall from a helicopter but is kidnapped by Chinese doctors who replace his heart with an artificial one that only lasts 60 minutes. He escapes with an external battery pack and seeks out Doc Miles, a mafia surgeon, who explains the situation. After destroying his external battery, Chev must find ways to charge his internal battery using friction and electricity. He tracks a man named Vang to a strip club, where he finds his girlfriend Eve working as a stripper. He learns that a mobster named El Hurón wants to kill him. Chev eventually learns that a Triad leader, Poon Dong, wanted Chev's heart and it has already been transplanted into him. Chev travels to Catalina Island, where El Hurón reveals he is the brother of two men Chev killed. El Hurón keeps Ricky Verona's head in a glass tank, kept alive to watch Chev be tortured. After a shootout, Chev grabs a high-voltage transformer to recharge, becoming set ablaze but gaining enough power to beat El Hurón to death. He then inadvertently sets Ria on fire while hallucinating she is Eve. In the end, Doc Miles replaces Chev's heart, and though it appears he died, Chev's eyes open in the credits.
