The story
Identical twins Elliot and Beverly Mantle are gynecologists who jointly operate a highly successful clinical practice in Toronto specializing in fertility problems. Elliot, the confident and cynical twin, seduces women who visit the clinic, then passes them off to the shy and passive Beverly, who remains unaware of the substitution. The equilibrium is disrupted when a patient, Claire Niveau, becomes emotionally attached to Beverly. As Beverly begins sharing Claire's prescription drug abuse, the twins' professional and personal lives begin to unravel, leading to a series of increasingly disturbing medical and psychological consequences.

Identical twins Elliot and Beverly Mantle operate a successful fertility clinic in Toronto. Elliot seduces patients and passes them to the passive Beverly. When Claire Niveau, a patient with a 'trifurcated cervix', becomes attached to Beverly, the twins' dynamic shifts. Beverly begins abusing prescription drugs with Claire's help. After Claire leaves town, Beverly falls into a drug-induced depression and paranoid delusions about 'mutant women.' He commissions bizarre gynecological instruments from an artist and attempts a surgery that ends in a violent accident, leading to the twins' suspension. Elliot locks Beverly in the clinic to 'synchronize' them with pills. Eventually, their roles reverse; Beverly cares for a despondent Elliot. During a mock birthday, Elliot volunteers to be killed to 'separate the Siamese twins.' Beverly disembowels Elliot on an examination table using a claw-like instrument. The next morning, Beverly realizes he killed Elliot during a drug-induced delirium. He leaves the clinic, calls Claire, and then returns to the clinic to die in Elliot's arms.
