She soon discovers that another man is being held captive in a neighboring room, who urges her to comply with the woman's 'game' and search for a key.
Full plot (contains spoilers)
Macy and her boyfriend Chase travel into a remote wooded mountain area in East Tennessee for a hike, where Chase intends to propose. While briefly separated, Chase encounters a large woman wearing a porcelain doll mask who attacks him and leaves him for dead. Macy is pursued through the forest and captured, awakening in a nursery in the woman's secluded house. The masked woman, known as "Dolly," forces Macy into disturbing, childlike roleplay and abusive treatment. Macy eventually finds a key in the basement, hidden in a doll with a human head. Feigning compliance, Macy frees the captive man, who is revealed to be Dolly's father; he tries to use Macy as a lure to escape, but Dolly kills him. Hearing Chase calling, Macy escapes to find him, but Dolly intervenes and kills him. Macy damages Dolly's mask, revealing a charred face, and manages to subdue her using a shovel and a previous victim's corpse. After Dolly decapitates a park ranger, Macy escapes in his vehicle, running Dolly over before fleeing, where she places her engagement ring on her finger.
