John Milton escapes from Hell after stealing Satan's gun, the Godkiller, to kill Jonah King, a Satanic cult leader who murdered Milton's daughter and his wife. King plans to ritually sacrifice Milton's infant granddaughter to unleash Hell on Earth. Milton discovers the ritual will take place in Stillwater, an abandoned Louisiana prison. He meets Piper Lee, a waitress, and takes her with him in her Dodge Charger. Satan's operative, the Accountant, pursues them to retrieve the gun. After a series of violent encounters, including a confrontation at a motel and a chase involving an RV, Milton and Piper are captured by King's cult. Milton is shot in the eye but awakens to kill the cult and pursue the RV. After receiving a 1971 Chevrolet Chevelle SS from a friend named Webster, Milton and Piper head to Stillwater. The Accountant captures Piper and forces Milton to surrender the Godkiller before he can battle King. Milton eventually slaughters King's men, and while King beats him, Piper fires the Godkiller. Milton grabs the gun and obliterates King and his soul. The Accountant retrieves the infant, allowing Milton to say goodbye to his granddaughter. Milton then agrees to return to Hell, drinking a beer from King's skull before being driven back by the Accountant in a 1957 Chevrolet Bel Air.
