A woman's premonition of a deadly highway pile-up saves a group of people, but they must then find ways to outsmart Death's design.
The story
One year after the explosion of Flight 180, college student Kimberly Corman is heading to Daytona Beach for spring break. On the entrance ramp to U.S. Route 23, she experiences a terrifying premonition of a massive pile-up caused by a logging truck. Kimberly manages to stop her car and alert others, successfully preventing several people from entering the road. However, as the pile-up occurs, she realizes that while she and others were saved from the immediate crash, they have entered a deadly game of survival. The survivors must now navigate a series of freak accidents as Death attempts to claim those who escaped the highway.

Following a premonition of a catastrophic pile-up on Route 23, Kimberly Corman saves several people, including a state trooper, a teacher, and a pregnant woman. While the others are saved from the initial crash, they begin dying in a sequence of freak accidents. Kimberly seeks help from Clear Rivers, a survivor of a previous disaster, who explains that Death is working backward to reclaim the survivors. The group, including the trooper Thomas Burke, attempts to find a way to stop the cycle. They encounter a mortician, William Bludworth, who suggests that 'new life' can defeat Death. They attempt to protect the pregnant woman, Isabella, whose birth is intended to create a loophole. Despite their efforts, several members of the group are killed in various accidents. In a final desperate move, Kimberly realizes she must die clinically and be resuscitated to break the cycle. She drives into a lake and is revived by a doctor. While she and Burke survive, the final scene reveals that even the 'new life' was not enough to stop Death, as a nearby child is killed in a tragic accident.
