Full plot (contains spoilers)
The film begins with a socialite's party where a man is mutilated by a puzzle box. Six years later, Riley McKendry finds the same box and, after a confrontation with her brother Matt, she solves a configuration, causing Cenobites to appear. Matt is taken by the box. Riley and her boyfriend Trevor track down a lawyer who is also taken by the Cenobites. At a mansion, Riley learns that the box offers 'gifts' from Leviathan in exchange for sacrifices. She encounters the mutilated owner of the mansion and eventually faces the Cenobites. After several companions are sacrificed or captured, Riley is forced to choose between sacrificing others or herself. She ultimately chooses to accept Matt's death to stop the immediate threat. Meanwhile, the mansion's owner, Voight, is transformed into a new Cenobite after his own failed attempt at power. The film ends with the box returning to its original shape as Riley and her remaining companion leave the mansion.
