The story
Following the events of the first film, three American art students traveling in Rome are convinced by a model to join her on a luxury spa vacation in Slovakia. While traveling by train and staying at a local hostel, the women are targeted by a sinister organization. They are eventually kidnapped and taken to a remote facility where wealthy clients pay to witness and participate in their torture. The film explores the harrowing experiences of the students as they navigate a deadly labyrinth of violence and exploitation in the Slovakian countryside.

After escaping the Elite Hunting Club, Paxton lives in seclusion until his body is found by the club's boss, Sasha Rassimov. In Rome, art students Beth, Whitney, and Lorna are lured by a model named Axelle to a spa vacation in Slovakia. At a hostel, their passports are sold to the Elite Hunting Club. Lorna is kidnapped and brutally murdered by a woman named Mrs. Bathory. Beth, Whitney, and Axelle are taken to a facility where they are subjected to various horrors. Whitney is mutilated and eventually beheaded by Stuart after he is pressured by the club. Beth is captured and faces Stuart, but manages to maim him before being 'freed' by Sasha, who gives her a tattoo. Finally, Beth finds and beheads Axelle, who was being toyed with by a group of children.
