An insurance investigator's reality begins to unravel as he tracks down a missing horror author whose stories may be bleeding into the real world.

The story

John Trent, an insurance investigator, is hired by a publishing house to find the missing horror author Sutter Cane. While investigating, Trent travels to the fictional town of Hobb's End, New Hampshire, where the boundaries between reality and fiction begin to blur. Accompanied by Cane's editor, Linda Styles, Trent discovers that the town's layout and residents mirror the author's books exactly. As they delve deeper into the mystery, they encounter unsettling occurrences that suggest the author's work is not merely fiction, but a catalyst for something far more dangerous and reality-altering.

The film begins with John Trent in an asylum, recounting his experience to a doctor. In a flashback, Trent is hired to find missing author Sutter Cane. He travels to Hobb's End with editor Linda Styles. They find the town perfectly mirrors Cane's fiction. Linda eventually confronts Cane at a church, where he reveals his stories were guided by 'them' and he shows her a manuscript that drives her insane, causing her to transform into a monster. Trent is forced to flee through a town of mutated people. He eventually finds himself in a confrontation with Cane, who explains that the mass belief in his work has dissolved the boundary between fiction and reality, allowing 'the old ones' to return. Cane tears himself apart, opening a portal. Trent escapes and finds himself on a road, eventually returning to the publisher where he learns the manuscript was already published. The film ends with Trent in the asylum, witnessing a global apocalypse of mutations and violence while watching a film adaptation of his own story.
