Chase, a paranormal fanatic, is interested in the local urban legend of "the Creeper," a creature that kills and eats hundreds of people every 23 years. While Laine believes she is pregnant and Chase plans to propose, the Creeper awakens to feed. After experiencing premonitions and picking up a mysterious shuriken, Laine becomes a target. The couple enters a raffle for a night at an abandoned plantation house converted into an escape room, joined by a crew and a local guide, Stu. As they approach the house through an 18th-century graveyard, the Creeper kills the cameraman and abducts Laine as Chase proposes. Laine awakens bound to a table where the Creeper pierces her abdomen. The group enters the house and feuds; Stu fires his pistol, attracting the Creeper, which then kills everyone except Stu and Chase. Laine frees herself and reveals she is pregnant and specifically targeted. They discover Madam Carnage, Lady Manilla, and others have been worshiping the Creeper and luring victims. They plan for Laine to lure the Creeper outside so Chase and Stu can push a weather vane onto it. Laine blinds the Creeper with her shuriken, and the falling weather vane impales it. A murder of crows consumes the Creeper, killing Stu in the process. The group departs, while elsewhere the Creeper regenerates and unleashes a devilish roar.
