A small mining community is plagued by a serial killer in mining gear who is believed to be a murderer thought to have died a decade prior.
The story
On Valentine's Day 1997, an explosion at the Hanniger mine in the small mining community of Harmony leaves only one miner, Harry Warden, alive; he is found to have killed the others to conserve oxygen. One year later, Warden awakens from a coma and begins a revenge killing spree, targeting patients and staff at the hospital where he was treated. While Tom Hanniger, his girlfriend Sarah, and their friends party inside the mine, Warden attacks them wearing mining gear and a gas mask. After a confrontation where some escape and Tom is left behind, the group is later terrorized by a masked assailant dressed as a miner.

In 1997, an explosion at the Hanniger mine in Harmony leaves Harry Warden as the sole survivor after he kills his fellow miners for oxygen. Tom Hanniger is blamed for the accident. A year later, Warden escapes the hospital and begins a revenge spree, leaving hearts in chocolate boxes. During a party at the mine, Warden attacks Tom, Sarah, Irene, and Axel; Sarah, Irene, and Axel escape while Tom is left with Warden. Sheriff Burke shoots Warden, but he retreats into the mine. Ten years later, Tom returns to Harmony to sell the mine. A masked killer begins murdering people in town, sparking rumors of Warden's return. Tom, Sarah, and Axel eventually find that Warden's grave is empty. The killer, who Tom believes is Warden, targets the group. It is eventually revealed that Tom has a split personality; he is the killer, having dug up Warden's gear to commit the murders. After a final confrontation in the mine, Sarah shoots Tom, causing an explosion. Tom survives the cave-in and escapes in the gear of a rescue worker.
