The story
Artist James Sunderland, a broken man struggling with the loss of his partner, receives a mysterious letter from his former lover, Mary Crane. The letter urges him to return to the town of Silent Hill. Upon arriving, he finds the town transformed into a desolate, fog-covered wasteland where he encounters terrifying figures and begins to question his own sanity. As he navigates the decaying environment, he encounters others and faces monstrous entities, all while trying to navigate the psychological toll of his past and the unsettling reality of the town's current state.

Years after a romance with Mary Crane ended, an alcoholic James Sunderland receives a letter from her and returns to Silent Hill. The town is abandoned and covered in fog and ash. He meets a woman named Angela and is attacked by a monster. The town transforms into an 'Otherworld' where he meets a man named Eddie and a girl named Laura. He encounters a ghoulish figure known as Pyramid Head. He meets a woman named Maria who looks like Mary; he recalls memories of Mary's involvement with a cult. In a hospital, Maria is killed by a nurse, and James encounters a moth-like version of Mary. He eventually realizes Maria is a doppelgänger and that he is delusional. He learns from Laura that she, Maria, and Angela are figments of his imagination. A final flashback reveals he actually found Mary months prior, and she died of a cult-induced illness; he smothered her to death. In a final vision, he chooses not to take her back to Silent Hill in a past memory, but to drive away with her.
