A self-help guru who fabricated his survival of a Jigsaw trap is forced into a real game to save his wife.
The story
Author Bobby Dagen, a self-help guru who gained fame by falsely claiming to be a survivor of the Jigsaw Killer's games, finds himself caught in a real game where he must save his wife. Simultaneously, John Kramer's ex-wife, Jill Tuck, informs internal affairs that rogue detective Mark Hoffman is the individual responsible for the recent Jigsaw games. As Bobby navigates a series of deadly tests in an abandoned psychiatric hospital to rescue his wife and several others who aided his lies, Hoffman continues his violent activities, leading to a confrontation involving internal affairs and the remaining survivors of the series.

Dr. Lawrence Gordon is revealed to have survived a bathroom trap years earlier. In a new game at a shopping center, Brad and Ryan are chained to a worktable with power saws to save their lover Dina; they eventually allow her to be bisected. Jill Tuck witnesses Mark Hoffman's escape and seeks help from detective Matt Gibson, offering to incriminate Hoffman. Hoffman abducts white supremacists, killing them in a junkyard, and also abducts Bobby Dagen. Bobby is trapped in a psychiatric hospital and must complete tests to save his wife, Joyce, and rescue his associates, though they all die in their traps. Bobby eventually reunites with Joyce after pulling out his own teeth. Gibson leads a SWAT team to Bobby's location, but they are killed by toxic gas. Gibson then discovers Hoffman's command center and is killed by an automatic turret. Hoffman kills everyone at the police station. For his final test, Bobby tries to hoist himself above a ceiling using hooks but falls, causing Joyce to be incinerated in a capsule. Hoffman executes Jill with a reverse bear trap but is subdued by three figures. The leader is revealed to be Dr. Gordon, who became Jigsaw's apprentice. Gordon shackles Hoffman in the same bathroom where he was once tested.
