A terminally ill man travels to Mexico for an experimental procedure, only to discover the operation is a scam and must exact his revenge.
The story
John Kramer, suffering from advanced brain cancer, is told he has only months to live. After hearing about an experimental treatment in Mexico, he travels to a clinic to see Dr. Finn Pederson. Upon arrival, he is treated by a team including Cecilia and Dr. Cortez. After the surgery, John is told he is cancer-free, but soon discovers the entire operation was a fraudulent scam. Realizing he has been deceived, John begins to hunt down those responsible, using his signature traps to punish them for their betrayal.
Full plot (contains spoilers)
John Kramer travels to a clinic outside Mexico City for a promised cancer cure. After the surgery, he discovers the operation was a sham and the medical staff were scammers. He kidnaps the taxi driver, Diego, and forces him into a trap. Simultaneously, his apprentice Amanda Young helps him abduct the other clinic staff: Cecilia, Mateo, Valentina, and Gabriela. The victims are subjected to various deadly traps. Valentina is forced to amputate her leg before being decapitated. Mateo is forced to drill into his skull. Gabriela is subjected to radiation. The plot thickens as it is revealed that some staff members were in on the scam. In a final confrontation, Cecilia and her accomplice Parker are trapped in a room filling with gas. Cecilia eventually kills Parker but is left alone in the room as John, Amanda, and a child named Carlos escape. The film ends with a glimpse of a former associate, Henry, being placed into a new trap.
