The story
New York-based pop star Skye Riley is preparing for a comeback tour following a public struggle with substance abuse and a tragic accident. While under the constant supervision of her mother and manager, Skye seeks out a former friend to obtain medication for a physical injury. This encounter leads to a terrifying event that leaves her haunted by disturbing visions and a mysterious, lingering presence. As her mental health begins to deteriorate, Skye must navigate a world where the line between reality and hallucination blurs, all while trying to manage the pressures of her public persona and the escalating threat of the entity that has now targeted her.

Following the death of Rose Cotter, the curse of the Smile Entity passes through several individuals before landing on Skye Riley, a pop star struggling with substance abuse. After a traumatic encounter with her friend Lewis, who is possessed by the entity, Skye becomes the new host. She is plagued by hallucinations of her past and current associates. Seeking help, she meets Morris, an ER nurse who understands the nature of the curse. He suggests a dangerous method to break it, but she initially refuses. After a series of terrifying encounters, including a hallucination of her mother and a realization that her friend Gemma is actually the entity, Skye eventually faces the entity's true form—a multi-mouthed, skinless creature. The film concludes with a meta-moment where Skye, during a concert, is fully possessed by the entity and performs a final, horrific act before a crowd of thousands.
