A psychopathic prison warden and his accountant attempt to create a massive human centipede to deter crime.

The story

At George H. W. Bush State Prison in the Southwestern United States, a cannibalistic and psychopathic warden named Bill Boss and his accountant, Dwight Butler, manage the facility. Following a series of incidents involving unruly inmates and a threatening prank call, Bill becomes increasingly volatile. To address the prison's high crime and violence rates, Dwight proposes a radical solution: suturing the inmates together to form a giant human centipede as a deterrent. Although initially skeptical, Bill eventually seeks out the director of the previous films to ensure the procedure is "medically accurate," leading to a tense atmosphere as the plan is set into motion.

At George H. W. Bush State Prison, a psychopathic warden named Bill Boss and his accountant, Dwight Butler, face a violent environment. After a prank call and a series of violent acts by Bill, Dwight suggests suturing inmates together into a human centipede to deter crime. Bill eventually agrees after consulting the director of the previous films. The inmates are tranquilized, and a 'Human Caterpillar' is also created from death row inmates. A girl named Daisy is sewn into the centipede. After the procedure, the Governor arrives; while initially horrified, he eventually declares the centipede as something "exactly what America needs." In the aftermath, Bill kills Dr. Jones, and later kills Dwight for trying to take credit for the idea. The film ends with Bill celebrating in a tower while the Star-Spangled Banner plays. An alternate ending depicts the events as a dream of Dr. Josef Heiter.
