Full plot (contains spoilers)

In 2009, John Jones, a medical student, becomes trapped in a small, 18-inch wide hole in Nutty Putty Cave while exploring with his brother, Josh. While Josh and a rescue worker named Susie attempt to help him, a veteran rescuer named Aaron takes charge of the operation. As John's physical condition deteriorates, he experiences hallucinations and shares memories of his life with Emily, his wife. During the rescue, a radio allows him to speak with Emily, who reveals she is pregnant. Despite a desperate attempt to pull him out using pulleys, a mechanical failure causes John to fall back into the hole. Emily is told he has passed away. However, John awakens and finds himself in a spiritual realm. He realizes he has died and encounters the spirit of his unborn son. He eventually finds peace, while in the physical world, Emily gives birth to a son whom she names after him. The film concludes with a note that the cave is sealed and Emily remarried within three years.
