Full plot (contains spoilers)

A young woman in Mexico is forced to burn another woman alive after a demonic voice asks her 'Truth or dare?'. In the present, college student Olivia Barron joins her friend Markie Cameron, Markie's boyfriend Lucas, and their friends Tyson, Brad, Penelope, and Ronnie on a spring break trip to Cancún. At a bar, they meet Carter, who leads them to the abandoned Rosarito Mission to play truth or dare. Carter reveals he was tasked with finding a group to play and warns them that they will die if they don't follow the rules. Back at school, Olivia begins to hallucinate classmates with demonic faces asking her to play. During the game, Ronnie is killed for failing a dare, Markie breaks Olivia's hand, and Brad is forced to come out to his father. The group discovers the story of Giselle Hammond, a woman murdered in Mexico. They eventually meet Giselle, who explains that a demon named Calux was summoned by a previous group. Giselle accidentally kills Penelope before committing suicide. The group learns of a 1960s massacre at the mission. Olivia is attacked by a possessed vagrant, and Lucas is forced to reveal his love for Markie. They visit Inez Reyes, the sole survivor of the massacre, who gives them a spell to stop Calux that requires the person who released him to sacrifice their tongue. Brad is killed during his dare. Olivia reveals her secret: she had encouraged Markie's father to commit suicide. They kidnap Sam (Carter's real identity) to perform the spell. Calux possesses Lucas, killing Sam and himself, then possesses Markie. Olivia records a video recounting the story and asks 'Truth or dare?' to expose the world.
