Full plot (contains spoilers)
After a student named Mason 'Horse' Raymore dies in a mysterious fire, transfer student Chrys Willet finds a skull-shaped Aztec death whistle in his locker. During a detention session, the students—Chrys, her cousin Rel, Dean, his girlfriend Grace, and a girl named Ellie—are present when the teacher, Mr. Craven, blows the whistle. This causes the students to be haunted by their own death apparitions. While the teacher is killed by a grotesque version of himself, the students learn from Mason's grandmother, Ivy, that the whistle's curse can only be avoided by transferring it to another person or by briefly dying and being revived. As the group is hunted, Grace is aged into an old woman and Dean is killed in a manner resembling a car crash. Rel attempts to sacrifice a local youth pastor, Noah, but the group refuses. Eventually, Noah is killed after accidentally taking the curse onto himself. While Chrys and Ellie believe they have escaped the curse, the film ends with a new student, Asha, finding the whistle in her locker and blowing it during a school assembly, dooming everyone present.
