Full plot (contains spoilers)
The gifted swordswoman Ochō Inoshika arrives in the Japanese port city of Kōbe to pay her last respects to the murdered head of the Ōgi clan—a godfather who once stood up for her and lost part of a finger for it. Shortly after her arrival, a case of mistaken identity lands her in the hands of shady gangsters, who first drug her, then abuse her and subject her to a brutal examination of her vagina. When the men realize their mistake—the gang uses the sexual organs of female couriers to smuggle drugs—they leave her alone. The humiliated victim then loses consciousness. When Ochō comes to, she is beside a woman’s corpse with a knife in her hand; the dead woman’s genitals have been mutilated. The woman is one of the victims in a brutal series of murders that has been shaking the metropolis for some time. The serial murders are to be pinned on Ochō, but the suddenly appearing former yakuza Jōji fortunately helps her escape. Humiliated and embittered, the swordswoman gets to the bottom of the mysterious murder spree while also trying to bring her tormentors to justice. As the story unfolds, the protagonist crosses paths several times with Jōji, a former prisoner and Ōgi yakuza who wants to kill Goda, the criminal organization’s illegitimate new leader. She also encounters Yoshimi, a nun and gang leader who is likewise hunting the unknown serial killer, whom she holds responsible for the murder of several gang members. Ochō also joins forces with a madam whose prostitutes are unwillingly serving as drug couriers. At some point, Ochō becomes involved with Goda, who secretly runs the lucrative drug trade to pay off outstanding gambling debts. Ochō eventually uncovers the bizarre drug-smuggling ring. In this situation, Goda proves to be a cold-blooded despot who has his own underage niece murdered to preserve his position of power. Nevertheless, Jōji and Ochō expose him. At the end of the film, the various yakuza face off against a group of enraged women, who successfully fight their male oppressors. In the final scene, Ochō walks toward a stylized sunset, followed by a group of loyal female fighters.
